Lumbar spondylolisthesis occurs when instability in the spine causes a vertebra to slip forward. This condition is categorized based on the degree of slippage, ranging from mild (Grade I) to severe (Grade V). It most commonly affects the L4-L5 or L5-S1 levels of the lumbar spine.
Congenital (Dysplastic): Abnormal bone formation present from birth.
Isthmic: Caused by a stress fracture or defect in a vertebral segment.
Degenerative: Resulting from aging and wear-and-tear changes in the spine.
Traumatic: Due to an acute injury that causes instability.
Pathologic: Associated with diseases like osteoporosis or tumors.
Lower Back Pain: Often worsens with activity.
Sciatica: Pain radiating down the buttocks and legs due to nerve compression.
Stiffness: Difficulty bending or twisting the lower back.
Weakness or Numbness: Tingling or muscle weakness in the legs.
Postural Changes: Increased lumbar curvature or difficulty standing upright.
A thorough evaluation includes:
Clinical Assessment:
Physical examination to assess posture, range of motion, and nerve function.
Imaging Studies:
X-rays: Reveal vertebral alignment and slippage.
MRI: Shows nerve compression and disc health.
CT Scans: Provide detailed images of bone structures.
Non-Surgical Treatments
Physiotherapy:
Strengthening exercises to stabilize the spine and core muscles.
Stretching to improve flexibility and alleviate stiffness.
Exercise Physiology:
Functional training tailored to your activity level and goals.
Programs to enhance overall mobility and reduce pain.
Advanced Therapies:
Shockwave Therapy: Stimulates tissue repair and reduces inflammation.
eMTT Therapy: Addresses chronic pain and supports healing.
Pain Management:
Heat or ice therapy for symptomatic relief.
Education on ergonomics to minimize strain during daily activities.
Medications
NSAIDs: For pain and inflammation relief.
Muscle Relaxants: To ease muscle spasms.
Topical Analgesics: For localized pain management.
Interventional Treatments
Epidural Steroid Injections: Reduce nerve root inflammation.
Facet Joint Injections: Provide targeted pain relief at the affected joint.
Surgical Options
Surgery may be considered in cases of severe slippage or persistent symptoms. Common procedures include:
Spinal Fusion: Stabilizes the spine by fusing the affected vertebrae.
Laminectomy: Removes part of the vertebra to relieve nerve pressure.
Reduction Surgery: Realigns the vertebra and stabilizes it with instrumentation.
Stay Active: Low-impact exercises like swimming or walking maintain spinal health.
Practice Good Posture: Supports natural spinal alignment.
Healthy Weight Management: Reduces pressure on the lower back.
Core Strengthening: Helps stabilize the spine and prevent progression.
Avoid Smoking: Smoking accelerates spinal degeneration.
